Form DS-11 – Application for a U.S. Passport (new applicants)
- First time applicants must submit a U.S. Passport Application in person
Form DS-82 – Application for a U.S. Passport by mail (renewal applicants)
- Renew by mail if all of the following is true:
- Most recent U.S. Passport is undamaged and can be submitted with the application
- Issued when customer was age 16 or older
- Issued within the last 15 years
- Issued in customer’s current name or customer can legally document his or her name change with original or certified copy of a marriage certificate or government-issued document evidencing the legal name change
